I HEREBY AGREE TO THE FOLLOWING

01 I am participating in classes or services during which I will receive information and instruction about meditation and/or yoga. I recognize that I may choose to do physical movement. I represent and warrant that I have no physical or mental health condition that would prevent my safe participation in meditation classes.

02 In consideration of being permitted to participate in the meditation and/or yoga classes, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I might incur as a result of participating in the program.

03 In further consideration of being permitted to participate in the meditation and/or yoga classes, I knowingly, voluntarily, and expressly waive any claim I may have against Shira Nicks and Shiralign LLC as well as any additional property owners for injuries or damages that I may sustain as a result of participating in classes or workshops held at the Studio on Online.

I HAVE READ THE ABOVE RELEASE AND WAIVER OF LIABILITY AND FULLY UNDERSTAND ITS CONTENTS. I VOLUNTARILY AGREE TO THE TERMS AND CONDITIONS STATED ABOVE.

I hereby release and agree to hold Shira Nicks and Shiralign LLC as well as any additional property owners harmless from, and waive on behalf of myself, my heirs and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Shira Nicks and Shiralign LLC.

I understand that this release discharges Shira Nicks and Shiralign LLC as well as any additional property owners from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Shira Nicks and Shiralign LLC. This liability waiver and release extends to the studio together with all owners and representatives of the Shiralign LLC.
